Pient Care Manager

Pay : $90,000-$100,000 based on experience

The Pient Care Manager plays a critical role in supporting both pients and the caregiving team, ensuring th every person receives compassione, highquality home health services. By guiding and empowering clinical staff, the Pient Care Manager helps cree a supportive environment where employees can grow, collabore, and deliver their very best work, ultimely enriching the care experience for every pient.

Through thoughtful coordinion of services, strong communicion, and adherence to professional and regulory standards, this leader nurtures a culture of excellence, safety, and trust. In this role, you influence quality outcomes, team success, and a meaningful difference in the lives of pients and the dediced professionals who serve them.

Wh You Need to Know:

Pient Care Manager Key Responsibilities

  • Provide clinical supervision to ensure pient care aligns with professional standards, agency policies, laws and regulory requirements
  • Coordine, plan, and monitor pient care
  • Supervise clinical personnel to ensure services are delivered appropriely and consistently
  • Maintain effective communicion with pients, caregivers, referral sources, and both field and office staff
  • Support quality outcomes by meeting departmental goals and participing in quality improvement initiives
  • Model professionalism, service excellence, and organizional values in daily work
  • Uphold compliance expections through required training, accure reporting, and cooperion with audits or investigions
  • Maintain a safe work environment by following and promoting safety protocols
  • Encourage staff development through ongoing coaching and participion in continuing educion

Pient Care Manager Qualificions

  • Registered nurse with current licensure to practice nursing in the practicing ste.
  • Previous experience in home care setting with two years management or supervisory experience, preferred.
  • Knowledge of accepted professional standards and practice, Medicare Conditions of Participion, and federal, ste, and local regulory requirements
