Oversee and promote the residents' health and wellness through regular visits with each resident, resident assessment, and ongoing communication with the resident care team.

Your Role in the Legacy Mission
As the Resident Services Director - Memory Care, you will champion health and wellness within our specialized memory care neighborhood. You'll partner with the Resident Services Director and Executive Director to ensure every care plan supports not just medical needs-but emotional connection, spiritual peace, and a sense of belonging.
Through your leadership and compassion, you'll help ensure Legacy Senior Living remains a trusted home where love and care never fade.
What You'll Do
Resident Health and Wellness
Meet regularly with each resident to provide health and wellness checks, addressing medical and emotional needs with sensitivity and care.
Complete comprehensive assessments prior to move-in, upon condition changes, and as required by policy and regulation.
Review all medical reports before move-in to ensure residents' needs can be met within the community's capabilities and regulatory standards.
Order medications and oversee medication administration processes in accordance with physician orders and state guidelines.
Ensure accuracy and completeness of medication documentation and administration records.
Collaborate with the Resident Services Director to develop and maintain individualized service plans for all residents.
Maintain open, ongoing communication with residents, families, physicians, and outside care providers.
Coordinate additional health or therapy services as needed and support transfers to or from hospitals or other facilities.
Provide compassionate counseling to residents and families navigating memory-related challenges.
Participate in on-call rotation for medical questions and emergencies.
Coordinate residents' dietary needs with the Food Service Director and ensure nutrition aligns with care plans.
Communicate changes in condition with the Resident Services Director, Executive Director, and physicians to maintain continuity of care.
Monitor infection control compliance and ensure a clean, safe, and nurturing environment.
Maintain accurate and confidential wellness files for all residents.
Training and Regulatory Compliance
Facilitate and document OSHA training to ensure all team members meet safety and compliance standards.
Lead and assist in training programs related to residents' health, wellness, and dementia care best practices.
Partner with the Executive Director to coordinate and track all state-required training for regulatory compliance.
Mentor and coach team members to promote excellence in resident-centered care and adherence to Legacy's mission.

Who You Are
A compassionate, patient, and skilled healthcare professional passionate about memory care
Licensed nurse (LPN or RN) preferred
Experienced in dementia or Alzheimer's care within senior living or healthcare settings
Knowledgeable in assessment, care planning, and regulatory compliance
A strong communicator and empathetic leader
Organized, dependable, and detail-oriented
Must pass background checks and meet all state and licensing requirements

Qualifications
- Current state nurses license (RN preferred)
- Minimum of one year experience in assisted living industry or health care
- Ability to respond during off-duty hours to questions and emergencies as scheduled
- Has a passion for Alzheimers care.
Knowledge Requirements
- Nursing practices and standards for older adults
- Maintain knowledge of federal and state laws pertaining to assisted living
Physical Job Requirements
To fulfill job requirements, employee must be able to perform key physical task for extended periods of time. When supporting, lifting or transferring residents, proper body mechanics and safety measures as prescribed by the residence must be used.
In a typical eight- hour day, employee will:
- Stand/walk up to six hours a day
- Sit up to four hours a day
- Frequently support up to 70 pounds
- Occasionally lift/carry up to1 5 pounds
- Frequently kneel, bend, and reach
- Secure proper assistance for transferring and lifting of residents as needed
