Salary: $20–$26/hr (based on experience), bonuses

Position Overview

We are seeking a highly motivated and compassionate Care Coordinator who can wear multiple hats, coordinate caregiver schedules and actively seek onboard new clients. This is a critical role for someone who is passionate about home care and will ensure our clients receive the highest standard of support.

Key Responsibilities

Coordination

• Match clients with appropriate caregivers, ensuring availability, skills, and compatibility.
• Maintain and manage the caregiver schedule, including handling changes, call-offs, and coverage gaps.
• Ensure compliance with agency procedures and Illinois non-medical home care regulations.

Sales & Client Acquisition

• Actively seek new clients through outreach, community engagement, referrals, and partnerships.
• Conduct intake consultations and present service offerings to prospective clients and families.
• Build and maintain relationships with hospitals, senior living communities, clinics, and local organizations.
• Track leads, conversions, and client satisfaction.

Qualifications

• High school diploma or equivalent required; associate or bachelor's degree preferred.
• At least 2 years of experience in admin or management in the home care industry, scheduling, or coordination within a home care or healthcare setting.
• Previous sales, intake, or community outreach experience in home care.
• Knowledge of non-medical home care regulations in Illinois.
• Excellent written and verbal communication skills.
• Tech-savvy – must be comfortable using scheduling software and mobile apps.
• Reliable transportation with valid driver’s license and insurance (travel required).
