As a Senior Patient Care Manager, you'll help guide branch-level clinical operations, oversee direct patient care activities, and provide leadership that ensures high-quality, compliant, and compassionate hospice services.
As a Hospice Patient Care Manager Senior, You Will:
Report directly to the Executive Director or Administrator
Lead and support a high-performing hospice program, ensuring excellent care delivery and regulatory compliance
Supervise, teach, evaluate, and develop nursing and clinical associates to promote clinical excellence and professional growth
Direct the scheduling and assignment of admit and evaluation visits and ensure clinical staff complete visits in a timely manner
Manage and oversee both clinical and administrative workflows such as documentation of referrals, DME pickups, Medicare eligibility, insurance verifications, and patient benefit tracking
Coordinate and lead Interdisciplinary Group (IDG) meetings, including patient reviews, care plan updates, and chart reviews
Ensure appropriate documentation and communication across clinical and operational teams
Monitor and reinforce adherence to company policies, procedures, and regulatory standards
Act as a key liaison between clinical teams, leadership, physicians, and other providers to ensure seamless, quality care
Qualifications - What You'll Bring:
Current RN license in the state of employment
Graduate of an accredited nursing program
Minimum of 3 years of direct patient care experience, with at least 2 years in hospice or home-based care
Proven ability to manage teams, clinical operations, and patient care coordination
Strong understanding of hospice principles, industry regulations (Medicare, Medicaid, JCAHO, ACHC), and best practices
Familiarity with managed care principles and human resource guidelines
Ability to lead with empathy and confidence while promoting quality, compliance, and operational excellence
Preferred Qualifications (Not Required):
Bachelor of Science in Nursing (BSN)
Prior experience in a clinical supervisory or program management role
CHPN certification (Certified Hospice and Palliative Nurse)
Proficiency in EMR systems and healthcare technology platforms

Salary Range: $90,000 - $108,000
