Our organization provides long-term residential care for three facilities in North County San Diego, California, catering to individuals with cognitive, behavioral, and physical disabilities.
Role Description:
The Case Manager holds the primary responsibility of coordinating care plans, conducting assessments, advocating for residents' needs, and collaborating with healthcare professionals, state-licensed officials, and authorized resident representatives.
Qualifications and Requirements:
- Experience in case management, assessment skills, and care coordination.
- Exceptional communication and interpersonal skills, both verbal and written.
- Proficiency in Microsoft Office applications (Word, Excel, Outlook, etc.).
- Ability to thrive in a fast-paced environment and effectively multitask.
- Minimum of a Bachelor's degree in Social Work, Psychology, Nursing, or a related field.
- Preferably possess Administrator certification or willing to obtain it within the first 6 to 9 months of employment.
- Experience with San Diego Regional Center (SDRC) and California Community Care Licensing(CCLD) is advantageous.
- Familiarity with medical and healthcare terminology is a plus.
- Flexibility in schedule is essential. Must be available to respond to texts and phone calls outside of regular business hours.
- Required to be physically present at the program during emergencies or urgent situations.
- Background check and fingerprint clearance are mandatory.
