Job Summary
Entry-level remote position supporting vulnerable populations by managing incoming calls and coordinating referrals to appropriate levels of care. Responsibilities include assessing eligibility for placement, communicating with referral partners and guardians, and providing trauma-informed support, including de-escalation when needed. Requires multitasking across systems, navigating EHRs, and collaborating with internal and external teams to ensure timely, accurate outcomes. Ideal for candidates with strong communication, organization, and problem-solving skills who can work independently in a fast-paced environment.

Minimum Requirements:

Education: Bachelor’s degree in a relevant field (i.e. Social Work, Education, Sociology, Psychology, Counseling, Human Relations, or Criminal Justice) preferred.

A high school diploma or equivalent with a minimum of two years of relevant experience (e.g., working with vulnerable populations, intake coordination, or crisis management may be considered.

Licensure/Certification: Valid drivers license and auto insurance. At least 21 years of age.

Experience: One or more years of related experience. Supervisory experience preferred.

Preferred Experience/Skills: Preferred candidates will have at least two years working with vulnerable or at-risk populations, along with strong technical skills in Microsoft 365, cloud-based platforms, and Electronic Health Records systems. Candidates should be comfortable managing multiple digital tools simultaneously while demonstrating excellent multitasking, organization, and time management abilities. Strong verbal and written communication skills are essential, as is the ability to work independently, problem-solve, and collaborate effectively in a remote environment. Experience with crisis intervention, de-escalation techniques, and trauma-informed care is highly valued, along with a high level of attention to detail when handling intakes and patient data.

Key Responsibilities

  • Provide respectful, inclusive interactions with clients, families, and community partners
  • Manage intake calls, walk-ins, and referrals; complete timely and accurate assessments
  • Coordinate placements or referrals to appropriate levels of care
  • Maintain strict confidentiality and accurate documentation in data systems
  • Communicate effectively with health plans, referral partners, and internal teams
  • De-escalate crisis situations using trauma-informed approaches
  • Collaborate with multidisciplinary teams to support care coordination
  • Navigate multiple systems (EHR, Teams, etc.) to ensure efficient workflow
  • Maintain clean, organized work and waiting areas
  • Attend trainings, meetings, and stay current on policies and procedures
  • Report incidents and follow organizational protocols
  • Deliver strong customer service while remaining professional under pressure
  • Support continuous improvement and contribute to a team-oriented environment
  • Work independently to troubleshoot issues and maintain efficient workflow in a fast-paced remote environment
  • Build positive relationships through strong communication, active listening, and interpersonal skills
  • Stay current on organizational policies, procedures, programs, and community resources
  • Maintain organized, professional workspaces and support daily operational needs
  • Participate in staff meetings, trainings, and ongoing professional development opportunities
  • Demonstrate professionalism, attention to detail, and commitment to diversity, equity, and inclusion

Additional duties may be assigned based on organizational needs and staff qualifications.

Physical & Work Environment

This role involves a mix of standing, walking, and occasional lifting, along with regular use of office equipment. You’ll work in a professional office setting with moderate noise levels and occasional exposure to outdoor conditions.
