Clinical Supervisor - RN / Registered Nurse - Home Care
The Clinical Supervisor is responsible for the clinical management of an assigned patient case load, supervision of field caregivers, office duties as assigned by the Clinical Director, Branch Director or designee, and following all applicable State/ Federal regulation, accreditation standards, and company policy/ procedure.
Essential Duties and Responsibilities include the following. Patient management includes but is not limited to performing.
Admissions
Recertification visits
Supervisory visits
Skilled visits
Hourly care
Managing physician orders
Care coordination between all caregivers and other entities involved in the patient's care
Caregiver Pre-Employment Meeting (PEM) and training as needed
Participating in 's Quality Assurance/ Performance Improvement (QAPI) activities.
Other duties as assigned
Minimum Requirements Education: Graduate of a Baccalaureate degree in nursing accredited by the National League of Nursing, or other 4-year college degree, strongly preferred. Current unencumbered license to practice as a Registered Nurse in the state of practice. Experience: At least two years of current nursing experience as an R.N. with a minimum of one year's experience in home health or a related field. Experience must include direct supervision, including hiring and firing within the last three years. Experience should emphasize problem solving skills in a patient care setting.
