LVN Case Manager - FULL TIME

LVN case manager works with new residents to assess their conditions and requirements along with the purpose of developing care plans customized to meet the specific personal needs of residents. Regularly evaluating these plans and updating them when needed are essential to the competent performance of a RN case managers duties. They need to impart education to residents and their families about the proper way of following the health plans.
Controls and administrates managed care cases of plan members admitted to the provider plan facilities and/or members utilizing services outside the provider plan facilities. Checks prior authorizations, performs concurrent reviews, indicates the appropriate level of care and utilization of services needed, develops discharge plans, and ensures quality cost effectiveness.
Develops a plan of care for residents from admission to discharge.
May develop assignments in coordination with the care partners and assist in supervising team members that are accountable for resident care and satisfaction.
Full-Time:
Mon - Fri 9am to 5pm
Pay:
* $30 - $34/hour *
Work Duties:
- Works with new residents to assess their conditions and requirements along with the purpose of developing care plans customized to meet the specific personal needs of residents;
- Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals and interventions;
- Coordinate the implementation of the plan of care and insure care coordination and case management for each patient. As applicable, apply specific criteria for admission and re-certification to establish appropriate levels of care and patients eligibility;
- Assesses residents needs at the time of move in and continually evaluates residents condition. Promotes the highest functional level of independence possible (keeping in compliance with state and federal regulations, and the communitys safety procedures)
Minimum Requirements:
Education
- Completion of Accredited Licensed Nursing Program (LVN) and current state license
Experience/Training
- Minimum of 2 to 4 years professional managerial experience in related field
- 2 years case management experience
- Clinical observation and assessment skills
- Proficient computer skills necessary to complete documentation, review electronic health records and communicate with team members
Certificates, Licenses, Registrations
- Current LVN license
- Current CPR certification
