Key Responsibilities
Coordinate and oversee individualized patient care services
Develop, implement, and monitor comprehensive care plans
Initiate and manage treatment interventions
Recruit, supervise, and support caregiving staff
Monitor service delivery to ensure quality and compliance
Conduct client intake and comprehensive assessments
Evaluate eligibility and admission needs
Provide ongoing follow-up and problem resolution for clients and families
Market Healthcare services

Qualifications
Bachelors degree (4-year) in Nursing, Healthcare Management, Social Work, or related field
Experience in care coordination, case management, or home health preferred
Strong supervisory and leadership skills
Excellent communication and interpersonal abilities
Proficiency in documentation and care planning
Ability to manage multiple priorities and problem-solve effectively
