Job Summary and Qualifications As a Transplant Patient Specialist, you will be responsible for utilizing a holistic and interdisciplinary approach to coordinate the care of recipients in the Solid Organ Transplant program. In addition, the Transplant Specialist may also be responsible for coordinating care from application through transplant and post care. You will collaborate with other disciplines and providers as it relates to the needs of the recipient for transplant. You will interact with the dialysis community as part of the transplant team as well as other community-based services. As the Transplant Patient Specialist, you will support transplant nurse coordinators and physicians with patient care management as it relates to transplant recipients, donors, and the transplant program. You will coordinate patient care under direction of the clinical team including, but not limited to, registering patients, coordinating patient testing and scheduling, reviewing clinical correspondence (i.e. lab results) for appropriate action, transplant database entry, monitoring patient lists for compliance with monthly blood specimens, obtaining patient medical records, and assisting the RN or other team members with patient care issues such as blood draw or vital signs. You will develop relationships with referring physicians, consultants, affiliates, and office staff. You will participate in providing patient specific care standards as directed and follows service excellence standards to ensure high levels of patient satisfaction. You will ensure that all required demographic, financial, referral/authorization, clinical, and other registration data is accurately scheduled, collected, verified, and communicated appropriately. You will schedule outpatient testing and consultations upon request of the RN coordinator and enter corresponding action into the medical record. You will schedule requested services for all NAMC sites. You will coordinate cases to ensure communication between departments. You will coordinate patient protocol steps for transplantation, including imaging, blood work, exams, and other required testing You will prepare patient charts; request necessary clinical information from dialysis units and/or physicians; prepare charts for revisits and re-evaluations by ensuring all tests ordered are retrieved and ready for review You will add missing forms to the chart as needed, such as progress notes and medication records, and labels each page with appropriate identifiers You will function as resource for patients and caregivers, referring providers, NAMC providers, and staff. You will obtain pending lab and/or test results at the direction of the RN coordinator. You will assist clinical staff with patient care needs, including follow-up phone calls, coordinating testing, drawing blood, scheduling tests/procedures, and securing insurance authorization or pre-certification. You will manage communication for transplant services.
