Position Description

To assist Admissions Director in meeting census goals and resident case mix according to budget demands. Assist with the smooth transition from referral location to facility via an informed and complete admission process. Represent the facility in a positive manner when dealing with community resources, prospective residents and/or family members.

Duties and Responsibilities

Act as back-up on resident admissions process (pre-admission/admission paperwork).

Conduct facility tours with prospective residents.

Verify prospective resident insurance and payer information as directed. Complete resident admission paperwork

Work with the interdisciplinary team and administration to promote and protect resident rights.

Provide information to the resident/family regarding Medicare/Medicaid, and other financial assistance programs available to the resident as directed.

Ensure or provide support and education to residents/family members/significant others to assist in their understanding of placement and facility issues in addition to referring them to the appropriate social service agencies when the facility does not provide the needed services.

Qualifications

Must possess, as a minimum, a high school diploma or equivalent.

Previous experience in long-term care environments is preferred.
