Job Description
Salary Range: $27.58 $36.67 Hourly

Provides recovery oriented services, teaching and mentoring to individuals with behavioral health needs. Actively outreaches to partner agencies and individuals in the community to engage individuals into needed services and supports. Facilitates and coordinates access to these services and supports within the community. Implements client treatment plans.
Responsibilities:
Case Manager/Skills Trainer helps formulate and carry out individual treatment plans, working in partnership with other team members and community partners. Assists clients with accessing local services and supports such as housing, social security benefits, education, physical health care etc. Provides active outreach and engagement to promote engagement in needed treatment services and community supports. Teaches life skills such as money management, emotional self-regulation, good interpersonal skills, self-advocacy and utilizing public transportation. Liaisons with community partners providing education on Options behavioral health services and facilitating access to these services. The majority of services are delivered in the community. Regularly drives agency vehicle and transports clients as required. Manages time and caseload effectively.
Qualifications:
Bachelors (BA/BS) degree in a human services field OR three years of experience providing services in a similar setting with individuals who have emotional and/or mental and/or developmental disabilities, OR a combination of education and experience that provides the desired skills, knowledge, and abilities required to perform the job.
Must be a QMHA on the Mental Health & Addictions Certification Board of Oregon (MHACBO) registry.
