Job DetailsJob Location: Family Health and Rehab - Wichita, KS 67209Position Type: Full TimeSalary Range: $37.00 - $42.00 HourlyJob Shift: AnyAs the MDS Coordinator, you will report directly to the RAI Director and Administrator. Your primary purpose is to complete resident assessments, create and review resident Care Plans, and submit MDS data, CAA, and Triggers data to governmental agencies. Ideally, you will be a problem solver, value hard work, good working relationships, have great communication skills, be organized, effective and strategic.
General Responsibilities:
Be aware of regulations and guidelines of Federal, State and facility
Make sure patients’ information is kept confidential
Conducts preliminary and comprehensive assessments of the needs of each resident
Develops a care plan based on the needs of the resident and goals to be accomplished
Adjusts the care plan as changes occur
Ensures care plan is being followed by all involved
Involvement in patients discharge and interim care plan meetings
Ensures the members of the nursing team are knowledgeable of patient’s responsibilities and rights, including the right to refuse treatment
Assist in reporting and investigating complaints from patients, families and staff
Submit data to CAA, Triggers and governmental agencies
Assures patients are treated with kindness, dignity and respect
May be required to work as a Change Nurse on occasion to fill gaps in staffing
Updates to insurance for authorization of continued skilled stay
Other duties as necessary
Working Conditions:
Medical environment
May require standing up or walking for up to 75% of the time
Required to use assistance when attempting to assist, lift or carry objects over 25 lbs.
Qualifications
Current state licensed practical nurse license required
3+ years of experience in the LTC or homehealth combined healthcare field
Experience with MDS preferred but not required
American Association of Post Acute Care Nursing RAC-CT Certification within 6 months of start date
Long Term Care Experience Required
