DESCRIPTION

SYNERGY HomeCare, Delray Beach is a CHAP-accredited Home Health Agency licensed by the State of Florida, providing non-medical services such as personal care, companionship, live-in and respite care for Palm Beach county residents. SYNERGY HomeCare is actively looking to hire an Intake / Scheduling Coordinator for the Delray Beach office location.

The Intake / Scheduling Coordinator is responsible for all office related activities of the company, including intake, patient care coordination and caregiver scheduling. The office operates in a paper-free environment and all documentation is only electronic.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Answer phones during normal business hours.

Receive and process incoming requests for new homecare services.

Schedule HomeCare Assessments for the Administrator/Supervisory RN in the client's home.

Make follow-up calls to prospects, document in scheduling software.

Provide superior customer service for clients.

Make quality calls to clients. Document in scheduling software.

Match and schedule caregivers based on skill level, availability and customer requirements.

Maintain employee files and keep them up to date with all the necessary paperwork.

Prepare for employee training/orientation.

Respond to time off requests, cancelled shifts, and call-offs by providing back-up caregivers and communicating with clients, as needed. Document all changes in scheduling system.

Requirements:

Strong communication, phone and administrative skills.

Proficiency with computers and ability to operate scheduling software such as AxisCare.

Knowledge of Outlook, Word, Excel and the widely used Adobe Acrobat.

Homecare or call center or staffing coordination experience is preferred.

Ability to maintain confidentiality.

Good auditory, visual ability; person will interface with computer, telephone as main job tools.

Ability to interact with a range of patients in a friendly, professional patient-oriented manner.
