Case Management Coordinator
Location: Miami-Dade County, FL (Field-Based) Schedule: Monday - Friday | 8:00 AM - 5:00 PM (Standard Business Hours) Compensation: $36.07 per hour + Full Benefits Travel: 75% Field-Based (Face-to-face member visits)
Role Overview
We are seeking a self-motivated, energetic, and highlyanized Case Management Coordinator to join our growing team. In this role, you will act as a vital link between members and the healthcare system, balancing telephonic coordination with consistent face-to-face engagement.
You will manage a diverse caseloadranging from supportive care to medicallyplex individualsto evaluate medical needs, determine appropriate services, and facilitate overall wellness through proactive intervention.
Key Responsibilities
Assessment & Planning: Conduct telephonic and face-to-face assessments to identify member health needs and social determinants of health.
Care Coordination: Develop and implement individualized courses of action, including coordinating with PCPs, skilled providers, andmunity resources.
Service Authorization: Manage prior authorizations and perform medication reviews to ensure members receive timely and appropriate care.
Field Engagement: Travel extensively (75%) within Miami-Dade County to meet members in their homes ormunity settings.
Documentation: Maintain accurate, tech-savvy records of all interventions and care plans in a fast-paced environment.
Required Qualifications
Bilingual Proficiency: Must be fully fluent in English and Spanish (verbal and written).
Location Requirement: Candidates must reside in one of the following Miami-Dade zip codes:
33126, 33125, 33015, 33018, or 33133
Transportation: Reliable transportation is required for daily field visits (Mileage is reimbursed to and from member homes).
Tech Literacy: Proficiency with digital platforms and case management software.
Preferred Skills
Exceptional ability to multitask and prioritize in a rapidly changing environment.
Strong verbal and writtenmunication skills.
Detail-oriented mindset with highanizational capabilities.

Pay Details: $36.07 per hour
