Job Description
CDRC Patient Disease Coordinator is responsible for coordinating inpatient/outpatient education for patients with Chronic Disease. Develop and apply evidenced based nursing practice standards to achieve optimal patient outcomes. Oversees evidence-based, individualized, effective care to patients and families in an environment of clinical excellence. Assists in maintaining a standardized patient education program regarding their disease, symptoms, and medications. Assists in the development of a standardized plan of care with measurable goals for patients with chronic disease. Initiate contact with patients in the acute care setting to provide initial/basic education and encourage follow up post discharge. Serve as an educational resource to hospital staff, patient and family, and outpatient primary care physician staff. Build relationships with community resources to connect patients with services. Implement and maintain education classes and coordinate content experts to deliver education. Maintain disease specific patient registry.
Education
Bachelors Nursing required
Masters Nursing preferred
Certification & Licensures
Virginia RN licensure required required
Ability to obtain licensure in West Virginia required
BLS Certification (Basic Life Support) - American Heart 'Healthcare Provider' (HCP) - AHA approved required *
- New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.
Qualifications
Ability to complete Telehealth certification required Knowledge and skills to develop adult education programs required
