Responsibilities:

• Coordinate the development, implementation, and ongoing evaluation of individualized care plans for our patients, ensuring that all services are patient-centered and priority-driven

• Collaborate with our skilled nursing staff, therapists, and other healthcare professionals to ensure seamless communication and effective treatment plans

• Review and maintain accurate and up-to-date records, including MDS assessments, care plans, and progress notes

• Participate in facility-wide quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient outcomes

• Provide exceptional customer service to patients, families, and healthcare providers, promoting a positive and respectful environment

• Stay current with regulatory requirements, industry developments, and best practices in MDS coordination, applying this knowledge to continuously improve patient care

Requirements:

• Curren Registered Nurse License (RN)

• Strong communication, organizational, and problem-solving skills

• Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and deadlines

• Familiarity with MDS assessments and care planning processes
