MDS Coordinator (LPN or RN) Orchard View Post Acute Orchard View Post Acute is looking for a dedicated and detail-oriented MDS Coordinator to join our team. If you are an LPN or RN who enjoys combining clinical knowledge with coordination and care planning, this is an opportunity to grow your career in a supportive and collaborative environment. Compensation $32 to $55 per hour, based on experience and qualifications What You'll Do As our MDS Coordinator, you will play a key role in ensuring accurate assessments and high-quality care planning for our residents. You will collaborate closely with nursing, therapy, and interdisciplinary teams to support positive outcomes and regulatory compliance.
