About the Role: The Ryan White Case Manager plays a crucial role in supporting individuals living with HIV/AIDS by providing comprehensive case management services. This position is focused on enhancing the quality of life for clients through resource coordination, advocacy, and support in navigating healthcare and social services. The case manager will work closely with clients to assess their needs, develop personalized care plans, and connect them with essential community resources. By fostering strong relationships with clients and collaborating with healthcare providers, the case manager ensures that clients receive holistic support tailored to their unique circumstances. Ultimately, the goal of this role is to empower clients to achieve better health outcomes and improve their overall well-being.
Minimum Qualifications:
- Bachelor's degree in social work, psychology, public health, or a related field.
- Experience in case management or social services, particularly with vulnerable populations.
Preferred Qualifications:
- Knowledge of HIV/AIDS and related health issues.
- Knowledge of Ryan White Part B Service Standards.
- Bilingual (English/Spanish) language proficiency.
Responsibilities:
- Conduct comprehensive assessments to identify clients' needs and develop individualized care plans.
- Coordinate and connect clients with appropriate community resources, including housing, food assistance, and mental health services.
- Advocate for clients within healthcare systems and social service agencies to ensure they receive necessary support.
- Maintain accurate and up-to-date case records, documenting client interactions and progress towards goals.
- Provide ongoing support and follow-up to clients, adjusting care plans as needed to address changing circumstances.
Skills: The required skills for this position include strong communication and interpersonal abilities, which are essential for building trust and rapport with clients. Organizational skills are vital for managing multiple cases and ensuring that all documentation is accurate and timely. Problem-solving skills will be utilized daily to navigate complex client needs and identify appropriate resources. Preferred skills, such as knowledge of HIV/AIDS, will enhance the case manager's ability to provide informed support and advocacy.
