Serenity Hospice PA is seeking a compassionate and skilled Registered Nurse Case Manager to join our dedicated hospice team in Lancaster, PA. As a Hospice RN Case Manager, you will:

• Coordinate and oversee the care of patients on your caseload

• Conduct nursing assessments and develop individualized care plans

• Ensure comfort-focused, compassionate end-of-life care

• Participate in on-call rotation as needed
