Purpose:
The Behavioral Health Care Manager (BHCM) serves as an integral part of the primary care team, establishing a therapeutic relationship with patients who meet criteria for enrollment into the Collaborative Care Model of Integrated Behavioral Health Program. The BHCM provides comprehensive, person-centered, community-focused interventions for enrolled patients. In collaboration with the primary care provider and the psychiatric consultant, the BHCM coordinates the physical and mental health care plans for an assigned patient caseload.
This position will be based in York County, specializing in serving patients with co-occurring substance use and mental health disorders.
Responsibilities:
Screen and assess patients for common mental health and substance abuse disorders.
Provide patient education about care plan needs and available treatment options.
Track treatment response and monitor patients throughout provision of care. Communicate response to care to primary care provider and psychiatric consultant in weekly case reviews
Support medication management with a focus on adherence monitoring, side effects and effectiveness of treatment.
Facilitate treatment plan changes as needed for patient care, in collaboration with both primary care provider and psychiatric consultant. Utilize weekly case reviews to gauge progress and seek additional case review times as necessary.
Document telephone, in-person, and consultative care encounters within the EMR.
Evaluates and utilize behavioral health registry within Epic EMR. Utilize registry and clinical decision support to identify new cases, as well as articulate progress with known patients.
Ensures a warm-handoff delivery and care coordination at all levels of care.
Coordinates referral patterns and community resource use for the service of the patient population.
Bachelor's Degree in a related field, including mental health counseling, marriage or family therapy, social work, nursing, or psychology.
Master's Degree preferred.
1-2 years providing service to behavioral health populations required.
Knowledge of evidence-based psychosocial treatments and brief behavioral interventions, including motivational interviewing and behavioral activation.
Knowledge of psychopharmacology as appropriate for the primary care settings preferred.
Must have own vehicle for daily travel in assigned service region.
Must maintain and provide proof of valid driver's license.
Must maintain own automobile coverage as required by law.
Licensure in good standing in the field of service preferred.
