Overview
HOPE Community Services, Inc. is a leading outpatient treatment provider in south Oklahoma City. We provide compassionate trauma informed care to individuals whose lives have been impacted by mental health and substance use disorders. We do this by providing compassionate and evidence-based treatment, aimed at improving the quality of life for those we serve.
We are currently seeking a dedicated and client-focused Case Manager to support individuals in accessing essential resources, maintaining benefits, and building the skills needed to remain stable in the community.
About the Position
The Case Manager provides advocacy, linkage, referral, and skills training to clients requiring frequent support to maintain community tenure. This role works closely with clients to address immediate needs, navigate benefits and insurance systems, and connect to ongoing services in a supportive, recovery-focused environment.
The ideal candidate is able to build strong, trusting relationships with diverse populations, communicate clearly and professionally, and manage multiple client needs with organization and attention to detail. This position requires the ability to work both independently and collaboratively, complete timely and accurate documentation, and support individuals with mental health and/or substance use challenges in a compassionate and professional manner.
Key Responsibilities
Coordinate services and advocate for clients and their families to ensure access to needed resources
Assist clients with obtaining and maintaining health insurance coverage, including Medicaid and other benefits
Assess eligibility for Medicaid and other benefit programs; educate clients to support informed decision-making
Monitor and support client eligibility for Social Security, Medicaid (Title XIX), Medicare, QMB, and other services
Maintain accurate and up-to-date client demographic and financial information
Provide face-to-face services, ongoing follow-up, and care coordination with program staff
Collaborate with internal teams to schedule appointments and share relevant client information
Deliver services in a manner that is recovery-oriented, trauma-informed, and welcoming
Maintain strict confidentiality in accordance with agency, state, and federal regulations
Meet HOPE's productivity expectations for service delivery and documentation.
Participate in supervision, training, and ongoing professional development
Provide services in community settings as needed, including client transportation
Requirements
Bachelor's degree in psychology, social work, or a related mental health field; or
High school diploma (or equivalent) with at least three (3) years of experience working with individuals with mental illness
Must obtain and maintain DMHSAS Case Management Certification
Must complete Therapeutic Options (or approved safety training) and annual updates
May be required to obtain First Aid/CPR and/or Wellness Coach Certification
Valid Oklahoma driver's license and acceptable driving record required for client transportation
