Responsibilities

Harbor Point Behavioral Health Center offers intensive residential treatment services within a secure setting. Our programs are designed to meet the needs of youth ages 7- 17 who are experiencing emotional, psychiatric and educational difficulties. In addition to our general psychiatric programs, we also offer specialty programs for youth with more serious challenges. Harbor Point and its programs are fully accredited by The Joint Commission and the Virginia Association of Independent Specialized Educational Facilities. We are licensed by The Virginia Department of Behavioral Health and Developmental Services and the Virginia Department of Education. We are a TRICARE®-approved and certified facility.

Position Scope :

The Case Manager serves as the facilitator for resident-centered services that promote coordinated, effective treatment by facilitating the communication exchange between the referral source, the physician/treatment team and payor and by managing timely transitions for the individual throughout their stay in the residential program.

Essential Functions :

  • Extract information related to resident needs and characteristics as well as funding availability from pre-admission process.
  • Facilitate admissions process to ensure exchange of information between resident/family, referral source, and treatment team.
  • Maintain an up to date file on each case assigned which includes funding information, concurrent stay reviews, and communication log/notes.
  • Facilitate problem-solving throughout residents stay regarding any barriers to resident's progress and timely discharge.
  • Assure that initial treatment plans are completed and faxed within 24 hours of admission.
  • Submit accurate written reports outlining clinical criteria for treatment, goals and objectives for continued stay, and current discharge plans in a timely manner.

Qualifications

Education: Bacherlor's Degree in appropriate Human Services field.

Experience: Must have minimum 1 year of experience working with adolescents in a psychiatric or substance abuse facility.

Physical Requirements: While performing the duties of this job, the employee is regularly required to stand, sit, walk, climb or balance and stoop, kneel, talk, and hear; use hands to finger, handle or feel; and reach with hands and arms. The employee must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close, distant, and peripheral vision, color and depth perception, and ability to adjust focus.
