Sonoma Post Acute is hiring a Director of Nursing (DON) to lead our clinical team in providing exceptional care to our residents. We're seeking a confident, compassionate RN leader with strong post-acute experience and a passion for mentorship, quality care, and team culture.

Responsibilities:

• Provide clinical oversight and leadership for the nursing department

• Ensure high-quality, person-centered care and regulatory compliance

• Supervise and support nursing staff, promoting education and growth

• Develop and monitor individualized care plans

• Work collaboratively with interdisciplinary teams and physicians

• Participate in quality assurance and improvement initiatives

• Serve as a key point of contact for residents and their families

Requirements:

• Current RN license in California

• Previous DON or ADON experience in a skilled nursing facility required

• Strong knowledge of Title 22, OBRA, and CDPH regulations

• Excellent leadership, communication, and organizational skills

• Ability to motivate and mentor a team in a fast-paced environment

• Passion for elder care and dedication to resident well-being
