MAJOR FUNCTION

Social Service case management and assessment of needs for indigent county residents for assistance through field, office or home interviews for eligibility in the Polk Healthcare Plan and referrals for community resources.

ILLUSTRATIVE DUTIES

  • Employees in this class must exercise reasonable initiative and independent judgment under general supervision.
  • Receives referrals, may make home visits, and interviews clients and or family members for assessment of need for multiple program services and community resources.
  • Reviews applicant's county eligibility status for medical assistance.
  • Evaluates financial, social, environmental, medical, family status, and related information, making proper disposition or referral of case on basis of eligibility.
  • Provides case management and counseling to assist clients with problem solving.
  • Counsels with applicants for referral to proper sources, developing community resources, and regarding county eligibility determination in accordance with existing county, state, and federal laws, rules and regulations.
  • Provides interpretations of rules, regulations and laws relating to the Polk HealthCare Plan and Health Insurance Portability and Accountability Act (HIPAA).
  • Makes referrals to various community resources throughout the county.
  • Acts in liaison capacity with medical personnel and other Social Service Agencies on the client's behalf.
  • Explains to the client the services available through the Health and Social Services Division.
  • Refers persons not eligible for county assistance and benefits in locating and/or securing aid and assistance from other area agencies.
  • Confers with supervisor on difficult and/or unusual cases.
  • Writes, gathers and reviews relevant client documentation to determine eligibility.
  • Enters required client data into the program's software system.
  • Prepares and completes case reports and correspondence, applicable and required forms and statistical reports.
  • Assists in special projects assigned to the section.
  • May perform crisis intervention; advocates for clients; and collaborates with community service agencies to provide information and services necessary to meet various socioeconomic needs in a variety of settings, to improve their situations and restore them to self-sufficiency in a community setting.
  • Provides client assessments and other related information to public officials and others for their use in determining appropriate courses of action relative to client needs and situations; coordinates with social services agencies, law enforcement, attorneys and judges to develop requirements to satisfy client needs; and attend meetings to exchange social services information.
  • Compiles data to be used in evaluating program performance, for reports, service provider payment, program documents, financial needs, and the budgeting process using manual and personal computer applications.
  • May be required to work occasional evenings and weekends.
  • Performs related duties as required.

  • Working knowledge of Federal, State, County and community social service programs for which most clients may be eligible including eligibility requirements, application procedures, and other relevant program policies.

  • Skill in interviewing clients to identify deficiencies in personal, legal, social, economic, and health needs.

  • May be required to lift, move or maneuver equipment and/or supplies that weigh up to thirty (30) pounds.

MINIMUM OUALIFICATIONS

  • Graduate of an accredited two (2) year college with major course work in Social Work, Sociology, Psychology or a related field and have a minimum of two (2) years social service interviewing, counseling or related case management experience.
  • Must possess a valid Florida driver' license at the time of employment.
  • 4 years of Social Services training and experience may be substituted for the minimum qualifications apart from the valid Florida Driver's license.

SPECIAL REOUIREMENTS

Must be able to provide own daily transportation. Minimum personal injury protection insurance is required to perform daily work assignments.

Must be available to work at various locations throughout Polk County.

The Health and Human Services Division is responsible for assisting in the Emergency Management Operations for Polk County in the event of an emergency. When circumstances warrant, such as a time of impending or declared disaster (i.e. hurricanes, tornados, flooding, etc.), the person filling this position will be expected to be available to participate in the Polk County Emergency Management Plan. This responsibility includes, but is not limited to, providing staffing coverage at various locations throughout Polk County, food and water distribution and/or any other duties which are deemed critical to assure the citizens of our County are provided with the necessary services during and after a disaster. The Emergency Management duties may require the employee to be away from their home and family for several consecutive days. Must be available for duty/participation during a disaster in the Special Care Shelter, Emergency Operations Center and/or at the office. May be exposed to communicable diseases. Must have a home telephone at the time of employment and maintain during employment as employee is subject to call out.

Must undergo a Level 2 (FDLE) screening submitted electronically before commencing work. Those obtaining this certification must maintain certification as a condition of employment.
