The purpose of the Neonatal Resource Services (NRS) Case Management program is to facilitate appropriate health care services (right care/right provider/right medication and right lifestyle) across the continuum of care as well as to monitor and evaluate options and services to meet an individual’s needs to promote quality, cost effective outcomes.

The goal of NRS is to perform timely discharge planning with the families in order to prepare the families for transition home and provide training to prevent readmissions. NRS works with families who have newborns who require more than routine newborn care or have intensive care needs. We deliver value for consumers of health care, payers or employers who purchase health care, as well as providers who deliver health care. Our Case Management program is a key initiative to help us realize our mission of empowering consumers to take ownership of their health: ‘To help people live healthier lives and to help make the health system work better for everyone’.

Primary Responsibilities:

  • Improve the health outcomes and satisfaction of consumers by providing consumer-centric care
  • Conduct initial and follow-up assessments within designated timeframes on infants identified as having high risk complex case management needs (assessment areas include clinical, behavioral, social, environmental and financial)
  • Holistically assess the infant’s current medical and family social circumstances to identify any gaps or barriers that would impact adherence with the prescribed treatment plan
  • Collaborate with family, caregivers, and healthcare providers to ensure that a well-coordinated treatment plan is established and continuity of care is maintained
  • Prioritize care needs, set goals and develop a plan of care that also addresses gaps and/or barriers to care and uses evidence-based practice as the foundation
  • Ensure quality of care issues are identified, acted upon and quickly resolved
  • Track the infant's health status and progress in achieving clinical and caregiver goals
  • Provide education, information, direction, and support related to identified care goals
  • Document assessments, interventions, and follow-up on Case Management activities
  • Regularly assess the effectiveness and quality of services provided to patients by analyzing outcomes (clinical, functional, and financial)
  • Maintain a focus on timely, quality customer service
  • Maintain a focus on the customer service through policy and program decisions and consider impact of these activities on the members
  • Facilitate problem resolution with members, providers, and other agencies or entities as needed
  • Promotes cost-effective health care consistent with adding value for consumers, customers and business
  • Serves as consumer advocate and maintains consumer privacy, confidentiality and safety
  • Adheres to ethical, legal/regulatory and accreditation standards
  • Participates in special projects, initiatives, and other job duties as assigned

Required Qualifications:

  • Unrestricted Compact RN license in your home state
  • Case Management Certification (CCM) within 24 months of hire if no prior case management experience; within 12 months of hire with one year or more case management experience
  • 3+ years of clinical experience in a Neonatal Intensive Care Unit
  • Proficiency in software applications that include, but are not limited to, Microsoft Word and Microsoft Outlook

Preferred Qualifications:

  • Bachelor’s Degree or higher
  • Case Management Certification (CCM)
  • Case management/managed care experience
  • Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you’ll find a far-reaching choice of benefits and incentives. The salary for this role will range from $58,800 to $105,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
