Type of Opportunity: Full Time (1.0 FTE/40 hours per week)
Work Schedule:M-F 8 hour days (1 hour lunch) 70% in office 30% in field.

This position is eligible for the Northern New Mexico differential of $4.00 per hour. (This differential may be adjusted or discontinued based on business requirements.)

Responsibilities:
  • Assess patient needs, including physical, mental, financial, and spiritual aspects

  • Provide skilled nursing care in alignment with physician/APC care plans

  • Coordinate patient care through collaboration with caregivers, multidisciplinary teams, and physicians

  • Adjust interventions as needed based on patient responses and updated orders

  • Evaluate effectiveness of interventions at each visit

  • Develop and implement hospice care plans while ensuring compliance with regulations

  • Maintain productivity standards for patient visits and caseload management

  • Communicate visit schedules and any changes to patients in a timely manner

  • Guide newly hired nurses and students, serving as a mentor and resource

  • Ensure documentation is accurate, timely, and compliant with agency policies

  • Delegate appropriate care tasks to LPNs and CNAs

  • Participate in quality improvement initiatives and performance improvement projects

  • Take on leadership responsibilities as required by the department

  • Provide holiday, weekend, and evening coverage as needed

  • Perform additional functions as required for patient care and agency operations

  • Lead the process for patient referrals and intake for admissions, evaluations, and consultations

  • Facilitate in-person goals of care conversations and explain hospice benefits to patients and families

  • Assist with staffing and scheduling new patient visits

  • Arrange transportation, medication orders, supplies, and equipment as needed

  • Build and maintain relationships with care coordinators, discharge planners, nursing homes, assisted living facilities, hospitals, physicians, and other referral sources

  • Serve as a liaison with hospitals, foundations, community groups, and other healthcare providers

  • Conduct admission visits, focused visits, and PRN visits in the community as needed

Qualifications:
  • Associate Degree in Nursing
  • Registered Nurse Licensure - Compact/Multi
  • Minimum one year in direct patient care setting or home health care.
  • Valid Driver's License
  • BLS required at time of hire
