The Clinical Manager provides direct supervision of an interdisciplinary team of field staff and coordinates activities of the health agency to provide quality care according to established best practices, federal and state regulations and Agency policies. The Clinical Manager provides oversight of all patient care services. Oversight includes: coordinating patient care, coordinating referrals, ensuring that patient needs are continuously assessed; and ensuring the development, implementation and updates of the individualized plan of care.

Responsibilities:
  1. Provides direct supervision of an interdisciplinary team of field staff (RN, LPN, PT, OT, SLP,MSW, HHA) in all aspects of the clinical process, making patient and personnel assignments.
  2. Interprets and executes Home Health Agency policies necessary to achieve the delivery of high quality, safe and cost effective patient care.
  3. Reviews and analyzes the current delivery of care, and plans and proposes modifications in this delivery to assure quality and cost effectiveness ensuring proper coordination of care and referrals.
  4. Assists agency staff in the planning and delivery of patient care.
  5. Participates in the Quality Improvement Program that includes QAPI, risk management and concurrent record review.
  6. Evaluates the performance of assigned staff.
  7. Undertakes corrective (disciplinary) action as appropriate.
  8. Collaborates with the Director of Patient Services in establishing goals and objectives for the Home Health Agency.
  9. Interacts with community agencies on behalf of VNSW or as a resource person for patients in the community.
  10. Participates in the orientation of new staff and for the continued professional growth of assigned personnel.
  11. Interviews and recommends the hiring of professional staff as requested.
  12. Remains conversant with the latest standards of practice and regulations.
  13. Prepares work schedules for assigned staff.
  14. Maintains knowledge/current information with CMS regulation and accrediting bodies in regard to clinical operating standards for home health care.
  15. Uses leadership position to serve as a resource to all staff regarding performance improvement an all areas to include – customer service, appropriate documentation with a focus on OASIS accuracy and timeliness, plan of care development and implementation and overall conduct and job performance.
  16. Complies with the Agency’s Corporate Compliance Policies and all laws, regulations, and standards of conduct established by federal and state governments.
  17. Performs all other duties as assigned. Clinical Coordination
  1. Assigns and oversees the Team’s caseload of patients through individual case manager assignments and caseload review. Assure that patient and family needs are continually assessed, prioritized, and met in order to achieve desired clinical outcomes.
    • Uses the principles of Patient-centered, family-focused care to interact with patients, families, physicians, and other care providers to identify agency strengths and weaknesses and intervene as necessary to provide for achieving desired patient outcomes. Assures the development, implementation, and updates of the individualized plan of care for patient.
    • Assures that all Agency staff members deliver respectful care consistent with the patient/family’s home, cultural, and environmental circumstances.
    • Collaborates with appropriate team members to develop and implement patient specific plans of care.
    • Promotes the development of care and practice standards and clinical pathways using benchmark/best practice markers and current research findings.
    • Assists with the establishment, monitoring, and maintenance of the clinical documentation system that makes evident patient/family needs, interventions, and progress in accordance with clinical, quality, and regulatory requirements. Assures that all required documentation is available in the electronic record as per agency policy.
    • Observes and promotes patient and family involvement in care planning, team conferences, and other mechanisms.
    • Conducts regular team conferences with other home care disciplines to promote information exchange, determine care assignments, and to promote integration of care plan.
  2. Participates in field visits and conducts point of care (POC) supervisory visits to supervise staff in the area of responsibility. POC visits should be conducted according to the Agency Policy on Field Competencies and occur at least twice annually for all field staff. May also visit customers to follow-up on risk management or satisfaction issues.
  3. Monitors ongoing patient care, through care conferencing and review of documentation, to determine that educational and discharge planning needs meet the requirements of the Conditions of Participation and are planned and implemented to ensure the highest quality care for all Home Health Aide patients at all times.
  4. Values and incorporates the results of, and techniques/principles associated with, scientific inquiry and quality management in professional practice.
Service Management
  1. Assures compliance with all local, state, and federal laws regarding licensure and certification of agency personnel and maintains compliance to CHAP Home Care standards. Knows and holds staff accountable for care based on contractual and reimbursement regulations/requirements.
  2. Develops, knows, interprets, and implements policies, procedures, safety practices, and emergency plans for the safe and effective delivery of home health care services provided.
  3. Ensures referrals for all related disciplines are made as indicated by comprehensive assessment and patient condition.
Personnel Management and Development
  1. Orients, counsels and retains Home Care staff. Brings counseling concerns, including recommendations for disciplinary action, to the attention of the DPS on a regular and ongoing basis.
  2. Assures the clinical staff comply with supervision of HHAs no less than every14 days and assures staff is familiar with patient plan of care and assesses the HHA’s ability to demonstrate initial and continued satisfactory performance in meeting Plan of Care for the patient and addressing any HHA competency issues.
  3. Completes at least two (2) evaluative field visits with assigned staff ( as part of the annual evaluation) assessing OASIS, Wound Care and other required clinical competencies.
  4. Reviews SHP results on a quarterly basis and works with Quality team and field nurse to develop clinical practices that will have a positive effect on quality and patient satisfaction.
  5. Assists staff to demonstrate interpersonal, cognitive, technical, and moral competency consistent with Agency values and mission.
    • Assists with the establishment of an orientation program for clinical staff. Teaches components of the program as requested by the Director of Staff Education.
    • Assures that all staff members are individually competent and have opportunities for individual and service-oriented growth.
    • Collaborates with individual staff members to establish and evaluate personal/professional Performance objectives. Conducts performance appraisals on at least an annual basis (six months for new staff).
  6. Knows and channels individual and group motives towards the achievements of Clinical and Agency outcomes.
    • Manages by planning and priority setting, organizing work, keeping daily operations functioning, setting frequent, attainable agendas/goals, making decisions based on mission and services.
    • Promotes workable unity between/among care providers, support staff, and consumers by creating an environment which promotes trust, risk-taking, and open communication. Seeks to diminish conflict by addressing issues in a matter-of-fact manner and by listening to all sides. Makes final decisions in a principle-centered manner and shares rationale for decision-making.
    • Participates in Research projects.

Qualifications:
  1. Current license and registration to practice as a RN, PT, OT, ST, MSW, MD or Audiologist in the State of New York.
  2. Bachelor of Science degree or equivalent for other disciplines, with a minimum of two (2) years of satisfactory experience as a Community Health Nurse in a Certified Home Health Agency or Long Term Care Program preferred.
OR
  1. Four (4) years of satisfactory experience.
  2. Demonstrated competence in their area of expertise.
  3. Knowledge of Home Care Home Base Clinical Software preferred.
  4. Adequate written and oral communication skills, including computer literacy and proficiency in Microsoft Word and Excel.
  5. Demonstrated leadership attributes.
  6. Professional liability insurance in the amount specified by VNSW.
  7. Automobile in good working order and auto insurance in the amount specified by VNSW.
  8. Valid Driver's license
Salary: $120,000 to $130,000
