Position Overview
We are seeking a highly motivated and compassionate Care Coordinator who can wear multiple hats, coordinate caregiver schedules and actively seek onboard new clients. This is a critical role for someone who is passionate about home care and will ensure our clients receive the highest standard of support.
Key Responsibilities
Coordination
- Match clients with appropriate caregivers, ensuring availability, skills, and compatibility.
- Maintain and manage the caregiver schedule, including handling changes, call-offs, and coverage gaps.
- Ensure compliance with agency procedures and Illinois non-medical home care regulations.
Sales & Client Acquisition
- Actively seek new clients through outreach, community engagement, referrals, and partnerships.
- Conduct intake consultations and present service offerings to prospective clients and families.
- Build and maintain relationships with hospitals, senior living communities, clinics, and local organizations.
- Track leads, conversions, and client satisfaction.
Qualifications
- High school diploma or equivalent required; associate or bachelor's degree preferred.
- At least 2 years of experience in caregiving, scheduling, or coordination within a home care or healthcare setting.
- Previous sales, intake, or community outreach experience is highly preferred.
- Knowledge of non-medical home care regulations in Illinois.
- Excellent written and verbal communication skills.
- Tech-savvy – must be comfortable using scheduling software and mobile apps.
- Reliable transportation with a valid driver’s license and insurance (travel required).
