JOB TITLE: Patient Navigator

JOB SUMMARY: The Patient Navigator works with a designated population to provide access to Health Care Providers to enable them to have quality medical and social support services. The Patient Navigator will create and implement an individual, multi-session, and interventions to connect patients with the Medical Care Team. The Patient Navigators will inform, coach, and assist individuals in a safe, empowering, personal manner. The Patient Navigator will be the liaison to the providers to assist in condition-specific counseling and referral services, as needed.

DUTIES AND RESPONSIBILITIES:

  • Build an effective working relationship to help clients/patients develop a rapport with the provider. Which will aid in a step-by-step plan to accomplish personal goals using an approved session plan.
  • Assist with patient appointments as necessary, including appointment reminders for initial diagnostic and/or follow-up appointments.
  • Facilitate structured client sessions conducted until the client links to medical care.
  • Assist patients with obtaining necessary medical records (e.g. films, pathology reports, etc.) for consultation appointments.
  • Work with Providers, Care Coordinators, nurses, and case managers to improve patient compliance with health care recommendations.
  • Coordinate and link each client/patient to available community resources, including medical care, medication coverage, and other health care required services.
  • Verify that medical care and other services are accessed.
  • Develop close working relationships with Care team members.
  • Adhere to strict procedures of confidentiality, release of information and data security.
  • Enter client/patient data and preliminary eligibility information into organization and program data collection tools.
  • Maintain accurate and complete participant records, tracking contact, and connections.
  • Assist with program special events.
  • Re-engage patients that have been lost to care.
  • Ensure to strategically reduce gaps in care while increasing targeted quality measures.
  • Perform other duties as assigned.

Qualifications

PREFERRED QUALIFICATIONS:

  • Ability to manage large volumes of calls effectively.
  • Familiarity with Electronic Health Records (EHR) and scheduling software.
  • Knowledge and experience with infectious disease care and support services, outreach, health promotion, and/or similar or related fields preferred.
  • Demonstrated ability to assist clients/patients with diverse psychosocial needs.
  • Proficiency with Microsoft Office Suite required.
  • Understanding of privacy regulations like HIPPA and patient confidentiality.
  • Bilingual Skills preferred.

MINIMUM QUALIFICATIONS:

  • Education- High school diploma or equivalent (associate or bachelors degree preferred).
  • Experience- Minimum of two years experience working and/or volunteering in medical scheduling, healthcare administration, or a similar position is preferred.

TYPICAL PHYSICAL DEMANDS:

  • Occasional standing, walking, lifting, reaching, kneeling, bending, stooping, pushing, and pulling.
  • Occasional lifting and/or moving up to ten pounds.
  • Frequent sitting.
  • Proficient reading eyesight and vision for close work.
  • Ability to communicate by speaking and hearing in person or on the phone.
